Boeing 777 engine explosion and debris falling from the sky over Denver in USA

The Boeing 777-200 operated by United experienced a right-hand engine failure minutes after it took off from Denver, CO, USA, on February 20th, 2021. The engine inlet separated, and numerous parts of fell off in a residential area and damaged a number of homes. The aircraft stopped the climb at about 13000 feet, the crew declared an emergency and returned to Denver. It finally safely landed about 23 minutes after departure.
The flight was heading to Honolulu with 231 passengers and 10 crewmembers. The Boeing 777 was fitted with Pratt & Whitney PW4000 engines. There was no injury onboard and on ground.

This reminds me of Southwest Airlines Flight 3472. It's on Air Disasters, and a passenger nearly ejects out the window after the engine's debris onto the window, and was killed. The pilots of S.W.A Flight 3472 managed to land safely and those ain't just any pilots; The captain was a U.S. Navy pilot, even the F.O. They're recognized the pilots and met the U.S. President, also the flight attendants were recognized, too! The flight attendants became heroes.

Now Boeing is grounding 777's with the P&W engines! Boeing's troubles continue as their stock takes a hit. Need to know why this happened - a simple maintenance error?
