Washington, DC, USAThe Canadair CRJ-700 operated by A PSA Airlines on behalf of American Airlines took off from Wichita, Kansas, USA, for a passenger flight to Washington National,DC, USA. 60 passengers and 4 crew members were onboard. The CRJ-700 had been cleared to land by ATC and was on final approach to Washington National's airport descending through about 200 feet AGL about 0.6 NM (1km) before the runway threshold when the aircraft collided with a military Sikorsky UH-60 Black Hawk helicopter. Both the CRJ-700 and the helicopter fell into the Potomac River. All 64 occupants and 3 occupants from the helicopter were killed.

The helicopter involved in the collision was on a training flight and belonged to B Company, 12th Aviation Battalion, Joint Task Force-National Capital Region, based at Fort Belvoir, Virginia. At the time of the crash, it was flying about 200 feet above its designated altitude.

At the time of the collision, a single air traffic controller at the Reagan National Airport tower was overseeing helicopters and several planes—a task typically handled by two people.
