35 plane crashes in year 2010

List of the 35 airplane accidents that happened in year 2010 :

Airline : South East Airlines
Aircraft : Tupolev TU-154M
The Tupolev TU-154M operated by South East Airlines (formerly known as Daghestan Airlines) took off from Moscow, Russia, for a passenger flight to Makhachkala, Russia. 163 passengers and 9 crew were onboard. The plane crashed 30 minutes after takeoff, while trying to make an emergency landing. 2 passengers were killed, 19 people received serious and 37 people received minor injuries. The airplane departed from Moscow's Vnukovo Airport with 19.5 tons of fuel on board. The aircraft became airborne at 14:08L (i.e local time), at 14:16L at a height of 6500 meters (21000 ft) the flight data recorder recorded first fluctuations in fuel supply to all three engines, at an altitude of about 9000 meters (29500 ft) engines #1 and #3 (left and right engines) failed. The crew reported the loss of two engines together with navigation equipment, and requested to divert to Moscow Domodedovo about 80 km (43 NM) from their present position. Engine #2 (central engine) became unstable as well but was recovered and continued to run until after landing at 14:36L. The airplane glided to a hard landing on Domodedovo's runway 32R in freezing rain about 30 minutes after departure, went off the runway and impacted a small earthen mound. The plane split in three parts, but no fire erupted. The fuel at Vnukovo was analysed and was found in compliance with all required fuel quality standards. Other aircraft having been fueled from the same supplies have reached their destinations safely. Shortly after the crash, passengers started surfacing from the smoldering wreck as firefighters and medics arrived at the scene.

Airline : United Parcel Service - UPS
Aircraft : Boeing 747-44AF
The Boeing 747-400 freighter belonging to United Parcel Service (UPS) took off from Dubai, United Arab Emirates, for a cargo flight to Köln, Germany. 2 crewmembers were onboard. The plane crashed shortly after takeoff, killing both crewmembers. The plane crashed while attempting to land at Dubai International Airport, from where the airplane had departed about 45 minutes earlier. Shortly after takeoff, the crew declared emergency reporting a fire and smoke in the cockpit, and performed an emergency descent. They requested an immediate return to Dubai. Later they reported they were unable to read their instruments and were unable to change frequency asking for frequent updates on their altitude and speed from ATC. They were vectored for a straight in approach to Dubai's runway 12L. Being too high and too fast for landing they were offered to divert to Sharjah or join a right hand downwind for another visual approach to runway 12L. About 20 minutes after declaring emergency, the airplane impacted ground in an unpopulated area of Dubai outskirts, near a busy highway intersection south-east of Dubai airport. The airplane erupted in a large fireball when he hit the ground, and slid about 400 meters. Several buildings, apparently all of them under construction, have been set on fire by the crash. The plane was loaded with mostly children's toys and plastic products.

Airline : Filair
Aircraft : Let 410UVP
The Filair LET L-410 was performing a passenger flight from Kinshasa, Congo, to Bandundu, Congo, with 18 passengers and 3 crewmembers. The plane crashed into a house about 2 km from Bandundu Airport. 20 occupants were killed in the accident, and one passenger survived. The airplane was destroyed, there was no fire. After an abortive attempt to land, the aircraft turned away and crashed towards the edge of Bandundu city, hitting an earthen house. The cause of the crash is unknown, but it is very likely that it has been caused by a human error that lead the plane to stall. The owner of the private airline Filair, a Belgian citizen, was at the controls of the Let-410 plane and was among those killed. 2 months after the accident, an African media reported the presence of a crocodile onboard that escaped from a suitcase, leading to a stampede. Passengers then rushed to the cockpit unbalancing the aircraft to a point, where control was lost. This story has then been reported by many international media. However, this is a very unlikely story: it is based on the testimony of an unidentified person. Nobody on ground remembers having seen a crocodile and the plane survivor has never told about such a story. In addition, among all the photos that were taken on the crash scene, no crocodile have been captured.

Airline : Aires
Aircraft : Boeing 737-73V
The Boeing 737-73V operated by Aires took off from Bogota, Colombia, for a passenger flight to San Andrés Island, Colombia. 125 passengers and 6 crewmembers were onboard. The plane crash-landed at San Andrés Island Airport. One passenger died. 119 people were injured (5 of them were seriously injured), and 11 unharmed. Thunderstorms were reported in the vicinity of the Carribean Island when the Boeing 737 was approaching to the San Andrés Island Airport runway 06. At the time of the accident the visibility was reported at 4000 meters (13000 feet), rain and winds around 15 knots. The crew did not report any malfunction of the airplane and no emergency call was radioed prior to impact. The plane touched down short of the runway, the landing gear was torn off on impact, and the Boeing 737 slid onto the runway on its belly as the fuselage fractured. The Boeing 737 broke in three parts. Firefighters quickly doused the beginnings of a fire on a wing. In interviews one of the pilots confirmed the airplane was struck by a lightning just short of the runway. The other did not recall a lightning strike, but a steep uncontrollable descent.

Airline : All West Freight
Aircraft : Fairchild C-123K
The Fairchild C-123 Provider cargo aircraft, operated by All West Freight originated at Palmer Wolf Lake Airport,AK, USA and was destined for Unalakleet, AK, USA. The plane crashed at the southern flanks of Mount Healy, Alaska, USA, about 1NM from the Denali National Park, about 180 miles north of Anchorage. The aircraft burst into flames on impact and disintegrated. Three crewmembers were onboard. All of them perished in the accident. The flight was to carry a large generator to Unalakleet. The flight was performed according to visual flight rules, no flight plan had been filed, the crew was not in contact with air traffic control and no radar services were provided. Numerous witnesses observed the plane flying slow and low (about 300-500 feet above the ground), flaps and gear were retracted, the engines were working. Cloud base was at about 3500-4000 feet. They observed the airplane pitch up to nearly vertical, roll left and nose dive toward the ground. A witness took two pictures, the first picture showed the airplane in straight and level flight, the second a few seconds later shows the airplane inverted in a near vertical descent just above the tree line. The airplane impacted sloped wooded terrain adjacent to the park highway at an elevation of 2158 feet, the wreckage area is confined within 250 by 300 feet (75 by 90 meters). A post impact fire consumed most of the cockpit area, fuselage, inboard sections of the wings (around fuel tanks), both flaps, and damaged a majority of the tail section and outboard sections of the wings. The post impact fire also started a small forest fire around the main wreckage and to an area adjacent to the accident site. The generator came to rest upright in the center of the wreckage consistent with its position in the cargo bay. The owner of All West Freight, who was also a pilot of the C-123, was among those that were onboard.

Airline : Airblue
Aircraft : Airbus A321-231
The AirBlue Airbus A321-200 was performing a passenger flight from Karachi, Pakistan to Islamabad, Pakistan with 146 passengers and 6 crew. The plane was on approach to Islamabad in poor weather conditions when the airplane impacted the terrain. All 152 occupants of the airplane perished in the crash. Monsoon rain and low visibility were prevailing during the Airbus A321 approach to Islamabad airport. The airplane had first approached the airport from the Southeast (suggesting an approach to runway 30), and ended by a go-around. The crew was about to try a second attempt from the Northwest (suggesting an approach to runway 12). While repositioning for the second approach the airplane impacted the Margalla Hills. The location of the impact is about 10 NM (18 km) North of the airport at a height of about 1000 feet (300 meters) above the airport. At the time of the arrival runway 12 was active. The airplane was performing an ILS approach to runway 30 circling runway 12. Initial investigation results suggest the airplane was flying too low in poor weather conditions and low visibility and incorrectly turned too late.

Airline : Lufthansa Cargo
Aircraft : MD-11F
The Lufthansa Cargo MD-11F was performing a cargo flight from Frankfurt, Germany, to Riyadh, Saudi Arabia. 2 crewmembers and 80 tons of goods were onboard. The freighter crashed while landing at Riyadh airport. The captain received minor injuries, and the first officer has been seriously injured. The plane is damaged beyond repairs. The MD-11F approached Riyadh airport, and hard-landed on runway 33L. The aircraft bounced on the first touchdown. This first touchdown was followed by two touchdowns. On the 3rd touchdown (g-load: 4.4g), the fuselage aft of the wing ruptured. This caused the fuel line to the #2 engine (central engine) and the APU to rupture. Fuel spread within the main gear area and ignited. The aircraft eventually ran off to the left of the runway, where the nose gear collapsed and the flight crew declared a Mayday. The airplane came to a stand still on soft ground just off the left hand runway edge. The two pilots exited the plane using emergency escape slides. Fire fighters were able to extinguish the fire that broke out in the centre area of the aircraft, front and rear portion were visibly not consumed by the fire.

Airline : Polish Air Force
Aircraft : Tupolev TU-154M
The Tupolev 154M passenger jet, operated the Polish Air Force, took off from Warszawa, Poland, for a passenger flight bound for Smolensk, Russia. 88 passengers and 8 crewmembers were onboard, including the Polish President Lech Kaczynski. The plane was destroyed when it crashed on approach to Smolensk Airport in poor visibility. All 96 people on board were killed. Weather conditions at Smolensk were reportedly poor, with heavy fog. The airplane was on its first approach, when he was advised by Air Traffic Controller of a very poor visibility of 400 meters due to fog and recommended to divert to another airport, but the crew however responded that they would give it one try and divert thereafter if they were not able to land. The final approach was flown on autopilot and autothrust. 18 seconds prior to impact with the first obstacle the terrain avoidance warning system warned the crew of the risk a collision with the terrain. 5 seconds prior to impact with first obstacles the decision was made to go around and the autopilot was disconnected. The airplane first impacted a barrier 40 meters (130 ft) left of the extended centerline of the runway and 1100 meters (3500 ft) before the threshold of the runway. At this distance the tri-jet should have been at a height of 60 meters (200 ft), the standard for a 3° glideslope. The third impact with an obstacle, an 8m-tall birch tree of 30-40cm trunk diameter contacted by the left hand wing, 840 meters (2700 ft) before the runway threshold and about 80 meters (260 ft) left of the extended runway centerline, led to the first break up of the aircraft in flight, the aircraft rolled inverted and impacted ground 5 seconds later. The airplane came to rest about 350-500 meters (1150–1600 ft) short of the runway threshold and about 150 meters (500 ft) left of the extended runway centreline. Impact forces were estimated in excess of 100G and were not survivable. Investigators noted the unusual crew interaction performed during this flight, due to the introduction of a navigator among the crewmember. No procedures were available for that type of crew combination and interaction, as the airplane is usually flown with a 3-man cockpit (captain, first officer, and engineer) and all training, documentation and manuals were set up only for a crew of three. The cockpit voice recorder also revealed that persons not belonging to the flight crew were on the flight deck about 20 minutes prior to impact. Rescuers that arrived on the crash scenes saw pieces of the plane scattered widely amid leafless trees and small fires burning in woods shrouded with fog. A tail fin with the red and white national colors of Poland stuck up from the debris. Three flights were to land at the Air Base in that period of time: the first was a Yakovlev YAK-40 carrying journalists accompanying Poland's president, which made a safe landing. The second was a Russian Ilyushin IL-76, which diverted after two unsuccessful approaches. The third was the presidential Tupolev TU-154M. Polish Aviation Accident Investigation Commission have released in July 2011 their final report. It concludes that the immediate cause of the accident was the descent below the minimum descent altitude at an excessive rate of descent in weather conditions which prevented visual contact with the ground, as well as a delayed execution of the go-around procedure. Those circumstances led to an impact on a terrain obstacle resulting in separation of a part of the left wing with aileron and consequently to the loss of aircraft control and eventual ground impact. Circumstances contributing to the accident were: Failure to monitor altitude by means of a pressure altimeter during a non-precision approach; Failure by the crew to respond to the PULL UP warning generated by the TAWS; Attempt to execute the go-around maneuver under the control of ABSU (automatic go-around); Approach Control confirming to the crew the correct position of the airplane in relation to the RWY threshold, glide slope, and course which might have affirmed the crew's belief that the approach was proceeding correctly although the airplane was actually outside the permissible deviation margin; Failure by LZC to inform the crew about descending below the glide slope and delayed issuance of the level-out command; Incorrect training of the Tu-154M flight crews in the 36 Regiment. The reports also identified that Unfavourable factors were: Incorrect coordination of the crew's work, which placed an excessive burden on the aircraft commander in the final phase of the flight; Insufficient flight preparation of the crew; The crew‘s insufficient knowledge of the airplane's systems and their limitations; Inadequate cross-monitoring among the crew members and failure to respond to the mistakes committed; Crew composition inadequate for the task; Ineffective immediate supervision of the 36 Regiment's flight training process by the Air Force Command; Failure by the 36 Regiment to develop procedures governing the crew's actions in the event of failure to meet the established approach criteria, using radio altimeter for establishing alarm altitude values for various types of approach, and distribution of duties in a multi-crew flight; Sporadic performance of flight support duties by LZC over the last 12 months, in particular under difficult weather conditions, and lack of practical experience as LZC at the SMOLENSK NORTH airfield.

Airline : Aviastar-TU
Aircraft : Tupolev TU-204-100
The Tupolev Tu-204-100 belonging to Aviastar took off from Hurghada, Egypt, for a ferry flight to Moscow, Russia. Eight crewmembers were onboard. The plane crashed during approach to Moskva-Domodedovo Airport, in a forest, about one kilometre short of the runway. Among the eight people onboard, four were seriously injured, and four others escaped without major injuries. Due to heavy fog on Moscow airport, the plane had to wait on a holding pattern. He then began an approach to Moscow airport, and attempt to land at night in fog and poor visibility. The crew did not report any failures, malfunctions, or the intention to make an emergency landing. But around one kilometre from touchdown the aircraft lost communication and disappeared from radar screens. Search efforts found the Tu-204's wreckage in woodland, where trees reached a height of up to 30m (100ft). There was no explosion or fire. First reports shows that the plan ran out of fuel while on holding pattern, and was not able then to reach the runway. Russian Aviation Authorities have banned Aviastar, with immediate effect, from conducting passenger transport.

Airline : CAA
Aircraft : Boeing 727-231F
The CAA (Compagnie Africaine Aviation) Boeing 727-200 performing a freight flight from Kinshasa, Democratic Republic of Congo, to Kananga, Democratic Republic of Congo, suffered a hydraulics problem shortly after takeoff prompting the crew to return to Kinshasa's Ndjili Airport. After landing, the airplane veered left off the runway and came to a stop with all gear collapsed. No injuries occurred. Because of the hydraulic leak, the captain decided to return to land to the longer runway at Ndjili. Heavy rain was prevailing, and there was water standing on the runway. After touch down, the airplane could not be slowed because the brake failed due to the loss of hydraulic pressure. The crew tried to use spoilers and thrust reverser, but there was no braking effect. The threshold of runway had been displaced and the now unused area of the former runway is being broken up and removed, leaving concrete piles on that section. The crew therefore tried to steer the airplane to the left hand edge around the works, but the left hand main gear went off the edge. There was an erosion at the left hand edge about 40cms deep causing the left main gear to drop below the concrete and the gear snapped off. The airplane skidded for about 300 meters on the right main gear, nose gear and left wing doing a 90 degrees pirouette in the process, at which point the right hand main gear snapped off. The right hand main gear ended up besides the left wing. Investigation showed that the tailskid had been removed by the maintenance team prior the flight. However, it is probable that the tailskid hydraulics lines were either badly locked or no plugs installed at all after the removal, leading to the loss of the hydraulic fluid when the hydraulic pumps were activated.
