32 plane crashes in year 2011

The Tupolev TU-134A operated by Altyn Air (alias Kyrgyzstan Altyn) took off from Bishkek, Kyrgyzstan for a passenger flight to Osh, Kyrgyzstan. 82 passengers and 6 crewmembers were onboard. The plane crash-landed on Osh's airport. 31 people received injuries. The airplane landed hard in dense fog. The hard landing caused the right main gear to collapse. This resulted in the right main wing separation and the airplane rolling on its back. A fuel leak on the left wing ignited a fire, which was quickly extinguished by airport emergency services.

The Boeing 737-400 operated by Sriwijaya Air took off from Jakarta, Indonesia, for a passenger flight to Yogyakarta, Indonesia. 131 passengers and crewmembers were onboard. While landing, the plane veered off the runway. The aircraft was evacuated, two passengers received injuries during the evacuation. The Boeing 737 had first aborted the approach to Yogyakarta due to weather conditions and diverted to Solo, however, again could not land in Solo due to weather and diverted to Surabaya. The weather conditions were reported improved in Yogyakarta and the crew decided to take off in order to land to Yogyakarta. However, strong rains were prevailing when the plane landed at Yogyakarta. After having touched down, the aircraft did not manage to stop before the runway end, veered left off runway and came to a stop on soft ground about 25 meters past and left of the runway end. The right main gear and the nose gear collapsed. The right engine and wing received substantial damage.

The de Havilland Dash 8-100 operated by Airline PNG took off from Lae-Nadzab, Papua New Guinea, for a passenger flight to Madang, Papua New Guinea. 28 passengers and 4 crew were onboard. The plane crashed about 12 NM (20 km) south of Madang, killing 28 occupants. 4 people onboard survived. The descent profile to Madang airport was steep because of the proximity of the Finisterre Ranges, a mountain range in north-eastern Papua New Guinea. The pilot-in-command was hand-flying the aircraft because the autopilot was unserviceable. He was maneuvering the aircraft visually to avoid cloud and thunderstorms. While descending, the overspeed warning horn sounded shortly followed by both propellers overspeeding simultaneously and exceeding their maximum rpm limit by about 60%. Seconds later, the crew declared an emergency and reported that both engines had stopped. The aircraft force-landed at 18 NM (33 km) south of Madang Airport, on sparsely timbered terrain on the north side of Buang River. The aircraft disintegrated due to impact with trees and ground and engulfed in flames. Rescuers rapidly localised the wreckage, which was on fire. The four survivors were the captain, the first officer, a flight attendant and a passenger.

The Casa C-212-100 Aviocar operated by Nusantara Buana Airlines took off from Medan, Indonesia, for a passenger flight to Kutacane, Indonesia. 14 passengers and 4 crew were onboard. The plane crashed while on approach to Kutacane. All 18 occupants perished in that accident. The plane took off for a 35-minute flight. The plane went missing while on approach to its destination airport. Rescuers localized rapidly the wreckage on a steep slope of about 70 degrees at about 6000 feet of elevation (1800 meters), but as the plane crashed in a remote and difficult terrain, they were able to reach the wreckage only two days later. The plane was caught by the forest canopy as it crashed, leaving it lying on the jungle floor. The aircraft had sustained substantial damage to its front section as well as the aft section at the belly, further to the impact with the terrain at about 140 knots. However, there was no evidence of fire, and some parts of the aircraft (wings, centre section) are largely intact. Nevertheless, none of the aircraft doors had been opened, and all occupants were found dead in their seats. At the time of the accident, a cloud ceiling of 520 meters/1700 feet was reported.

The Yakovlev Yak-42 operated by YAK Service took off from Yaroslavl, Russia, for a passenger flight to Minsk, Belarus. 37 passengers and 8 crewmembers were onboard. The airplane crashed right after takeoff, 1 kilometer away from its departure airport. Among the 45 people on board, 44 people were killed, and 1 survived with serious injuries. The plane failed to climb after lift-off, and impacted an antenna outside the airport perimeter. The aircraft reached a few tens of meters of height, and suddenly rolled left and impacted ground. The airplane came to rest about 1 km (0.6 NM) from the airport shattered into pieces, the tail section at the bank of Volga River and the fuselage in the river. The weather conditions were very good at the time of the accident, with few clouds, low wind and good visibility.

The CASA C-212 Aviocar belonging to the Chilean air force took off from Santiago, Chile, for a flight to the Juan Fernandez Islands, Chile. 15 passengers and 6 crewmembers were onboard. The plane crashed while approaching its destination. All the occupants have been killed. Strong gusts of wind and rain squalls prevailed at the time of the accident on the remote island. The aircraft circled twice over the runway before starting the approach. The plane then disappeared from radar screen during the approach, and crashed into the ocean. The wreckage has been found about 1 kilometer (0.6 NM) away from the landing strip. Juan Fernandez Islands are located 830 kilometres (450 NM) off the Chile’s Pacific coast.

The Boeing 737-210C/Adv (combi aircraft) operated by First Air took off from Yellowknife, NT, Canada, for a passenger flight to Resolute Bay, NU, Canada. 11 passengers and 4 crewmembers were onboard. The plane crashed shortly before landing at Resolute Bay. 12 occupants have been killed, and 3 survived with injuries. Fog and rain prevailed at the time of the accident. The plane crashed while approaching Resolute Bay. Its last reported communication was about 5 NM (8 kilometres) from Resolute Bay airport. The wreckage was found at around 1 NM (two kilometres) west of the Resolute Bay airport's landing strip. The Canadian Forces were conducting an exercise called Nanook 11, which includes the simulation of a major air disaster near Resolute Bay. The exercise was interrupted to allow the forces to join the search and rescue operation in progress. The First Air Boeing 737-200 had no part in any simulation exercises. The Boeing 737-210C/Adv is a combi aircraft: the plane can carry both passengers and cargo on the main deck. When it crashed, the aircraft was in a configuration with room for 34 passengers and 5 cargo palettes.

The Antonov AN-12 operated by Avis Amur took off from Magadan, Russia, for a cargo flight to Keperveyem, Russia. 11 crewmembers and 16 tons of cargo were onboard. The plane crashed while in cruise, in Russian remote far eastern region. All 11 crewmembers perished. The plane was en-route, when the crew reported a fuel leak and an engine fire and said its intention to return to Magadan airport. Radar contact with the aircraft was lost about 170 NM (315 km) northeast of Magadan. The wreckage has been located a few hours after the crash in the area where the radar contact was lost, 6 NM (10 km) away from Omsukchan. The plane was destroyed. The plane was owned by Komsomolsk-on-Amur aircraft building plant and had been leased to Avis-Amur. The plane was carrying 16 tons of food supplies.

The Antonov AN-24 operated by IrAero, took off from Chita, Russia, for a passenger flight to Blagoveshchensk, Russia. 31 passengers and 5 crewmembers were onboard. The crash-landed in Blagoveshchensk, injuring 14 people onboard. Heavy rain reduced visibility (1000 meters / 3300 feet) prevailed at the time of the accident. Cloud base were at 500 ft. The aircraft performed an ILS approach to runway 36 descended below the glide path and veered off course. As result the aircraft hit a tree about 75 meters (250 ft) ahead of and 250 meters (820 ft) to the right of the runway threshold, leading the plane to smash on ground. While skidding over terrain the aircraft turned nearly 90 degrees and came to a stop 285 meters (935 ft) to the right of the runway and about 450 meters (1500 ft) past the runway threshold. The plane came to a stop on a soft ground, and was badly damaged: the landing gear collapsed and the left wing shattered after tree impact. There is no fatality, but 14 people (among which the captain and the first officer) have been injured.

The Boeing 737-8BK operated by Caribbean Airlines took off from Port of Spain, Trinidad and Tobago, for a passenger flight to Georgetown, Guyana. 154 passengers and 8 crewmembers were onboard. The plane missed the landing and broke up. All the people onboard survived, but two passengers received serious injuries. Heavy rainfall prevailed at the time of the accident, which reduced the visibility. The plane touched down Georgetown's runway 06 too far away from the runway threshold, and could not stop by the time he reached the end. The plane overran the runway by about 330 feet (100 meters), broke through the perimeter fence, fell onto a perimeter road and broke in two. The front section of the plane separated just ahead of the wings.

The Boeing 747-400F operated by Asiana Cargo took off from Seoul, South Korea, for a cargo flight to Shanghai, China. Two crewmembers and 58 tons of cargo were onboard. The plane crashed into the ocean off Jeju Island, 67 minutes after it had taken off Seoul. The two people onboard were killed. The plane took off uneventfully, but then, while the B747 was enroute near Jeju Island (about 250 NM (463 km) South of Seoul), the crew reported the cargo in the hold had caught fire and they needed to divert to Jeju Airport, which was their only possible point of diversion. The crew's distress was clearly audible and increasing during the transmissions. The plane began to divert, but radar contact with the plane was lost while the plane was descending. Debris from the plane was found later in East China Sea, about 58 NM (107 km) west of the city of Jeju. The plane was completely destroyed. The cargo aircraft was carrying 58 tons of cargo including 0.4 tons of hazardous materials like Lithium batteries, paint, amino acid solution and synthetic resin. All the cargo onboard was compliant with the IATA (International Air Transport Association) regulation.

The Lockheed C-130H Hercules operated by the Moroccan Air Force took off from Dakhla, Morocco, for a military flight to Kinitra, Morocco, with an en-route stop at Goulimime, morocco (also known as Guelmim). The airplane carried 9 crew members, 60 troops and 11 civilians. It crashed during its approach to Goulimime air base, killing all 80 people onboard. The plane was preparing to land at Goulimime's military air base when it slammed into a mountain. The wreckage was found rapidly after the accident, in a mountainous area in south of Morocco, about 10 km East of Goulimime air base. The plane has been pulverised by the impact. The weather conditions were bad: dense fog – therefore low visibility - was prevailing at the time of the accident.

The Let L-410 operated by NOAR Linhas Aéreas took off from Recife, Brazil, for a passenger flight to Natal, Brazil. 14 passengers and 2 crewmembers were onboard. The plane crashed shortly after takeoff, killing all 16 people onboard. The twin-turboprop aircraft developed technical problems immediately after takeoff. The plane was in initial climb, less than 1 minute into the flight, when the crew declared an emergency and indicated they would attempt to return to the runway. Two minutes later, the crew indicated they could not reach runway 36 and that they would attempt an emergency landing on the beach of Boa Viagem. The Czech-made L-410 was seen steeply descending and crashed 20 seconds later and burst into flames. The plane has been pulverised. The airplane crashed in a vacant field near the beach, about 1 NM from the runway end.

The Angara Airlines Antonov AN-24 operating a flight on behalf of Tomsk Avia took off from Tomsk, Russia, for a passenger flight to Surgut, Russia. 33 passengers and 4 crewmembers were onboard. The plane developed problem in cruise and came down in the Ob River, close to Strezhevoy Airport, killing 6 passengers. The 31 other people onboard survived with various degrees of injuries. The aircraft was enroute at FL 200 (around 6000 meters in altitude) when the chips in the oil filter warning indication for the left engine triggered. The crew decided to divert to Nizhnevartovsk airfield. During the descent, a fire broke out in the left engine. The crew discharged both left engine halon bottles, but the fire did not stop. The crew then decided to perform a forced landing on a sandbank of River Ob. The plane approached the ground slowly, in a stabilized manner, and crash-landed. The tail separated from the fuselage during the landing. The plane eventually ended about 30 NM short of Nizhnevartovsk airfield.

The Boeing 727-100 operated by Hewa Bora took off from Kinshasa, Congo, for a passenger flight to Kisangani, Congo. 112 passengers and 6 crewmembers were onboard. The plane crashed on landing at Kisangani Airport, killing 74 people. 44 people survived. The weather conditions at the time of the accident were very poor, with heavy rain, low visibility, thunderstorms in the area reported. The plane missed its landing on Kisangani's runway and broke up coming to rest about 600 meters from the runway.

The Ilyushin IL-76 belonging to Silk Way Airlines and operated on behalf of International Security Assistance Force (ISAF) took off from Baku, Azerbaijan, for a cargo flight to Bagram Air Force Base, Afghanistan. 9 crewmembers and 18 tons of cargo were onboard. The plane crashed while on approach to Bagram's Air Force Base. All the nine people onboard were killed. The transport plane carried a total of 18 tons of cargo (automobiles and trailers). It was on approach to Bagram's Air Force Base, descending through 13000 ft (4000 meters) when radar contact was lost. At the same time a flash and fire were observed on a mountainside about 25 km from Bagram. No emergency call and no report about any anomaly had been received from the crew. The wreckage was located in a mountainous area, near Akhshay, at an altitude of 12500 ft (3800 meters).

The Tupolev Tu-134A-3 operated by RusAir on behalf of RusLine took off from Moscow, Russia for a passenger flight to Petrozavodsk, Russia. 43 passengers and 9 crewmembers were onboard. The aircraft crashed while on short of the runway while approaching its final destination. 47 occupants were killed, and 5 survived with injuries of varying degree (from medium to critical). The plane was on approach to Petrozavodsk airport when it clipped some trees and a high power line. The aircraft subsequently crashed on a highway, broke up and burst in fire. The plane came to rest about 390 meters to the right of the runway's extended centerline and 570 meters short of the runway threshold. Debris are scattered on the highway and in the nearby gardens of several private homes at the outskirts of the Besovets village. No home was hit. Weather reports indicated a poor visibility of about 2000 meters (6500 feet), rain and overcast cloud at 400 ft (120 meters). Petrozavodsk's runway 01/19 (former 02/20) is 2500 meters/8200 feet long. The airport offers non-precision NDB approaches to runway 01 and 19.

The Antonov AN-26 cargo plane, operated for DHL, took off from Port Gentil, Gabon, for a cargo flight to Libreville, Gabon. 3 crewmembers and 1 passenger were onboard. The plane crashed into the sea during its approach to Libreville airport. The 4 crewmembers sustained minor injuries. The aircraft was operated by Solenta Aviation Gabon, on behalf of the DHL Aviation express courier service. The plane had been given permission to land in the capital when the pilot reported there had been a hydraulic problem. The plane ditched into the sea just off the shore near Libreville Airport. The airplane came to rest submerged with the top of the tail sticking out of the water. All 4 occupants left the aircraft with minor injuries.

The Boeing 707-321B experimental aerial tanker plane, operated by Omega Aerial Refueling Services Inc, took off from Port Hueneme-Point Mugu NAS, CA, USA, for a test flight. The plane was expected to come back to its departure airport after the flight. 3 crewmembers were onboard. The four-engine plane crashed on takeoff, and has been destroyed. The three crewmembers sustained minor injuries. Omega Aerial Refueling Services Inc, was operating the airplane as a local public use flight. The flight crew indicated that they delayed their takeoff due to high wind conditions. Once the winds were within limits, they reported that startup was normal. Prior to take off, they decided to add 5 knots to their rotation speed, and power up slowly and as smoothly as possible. The captain was the flying pilot. The first officer called critical engine failure recognition speed (V1) reference speed at 141 knots, and rotation speed (Vr) at 150 knots. The captain rotated the airplane, and it then lifted off about 7000 feet (2130 meters) down the 11000-foot runway (3350 meters). About 20 feet (6 meters) above ground level, the number two engine (left inboard) throttle lever slammed back to the idle position. The airplane stopped climbing, and the captain adjusted the pitch slightly down to hold V2 speed. Witnesses observed the airplane lift off, and something shiny go up and over the left wing. They also observed a fireball on the left side of the airplane. The flight crew reported that the airplane began to drift to the left and descend. The captain lowered the pitch slightly and leveled the wings just prior to the airplane contacting the runway. He informed the other crew members that they were going to put it back down. He placed the throttle levers in the idle position, and activated the speed brakes. The airplane departed the left side of the runway surface; the crew reported a couple of impacts and then one final violent impact prior to the airplane coming to rest in a wetland marsh. The flight crew had to force the cockpit door open due to debris that had piled up against it. They noted significant damage from the forward galley aft, and a large fire in the vicinity of the left wing. They opened the left main cabin door, and deployed the slide. They exited into the mud, and made it to dry land and away from the burning wreckage. The airplane sustained substantial damage to the wings, fuselage, and empennage from impact forces and post crash fire. The main wreckage came to rest in a wetland marsh left of the runway overrun, and caught fire. Fire consumed the top of the cabin and the cockpit. The main wreckage consisted of the cockpit, cabin, right wing with the number three (right inboard) engine partially attached, empennage, and the inboard half of the left wing, which sustained thermal damage and was under water. Scattered debris aft of the main wreckage included the nose gear, remnants of the burned outboard left wing, right main landing gear truck, and number four (right outboard) engine.
