32 plane crashes in year 2011

List of the 32 airplane accidents that happened in year 2011 :

Airline : Airlines PNG
Aircraft : DHC-8-102
The de Havilland Dash 8-100 operated by Airline PNG took off from Lae-Nadzab, Papua New Guinea, for a passenger flight to Madang, Papua New Guinea. 28 passengers and 4 crew were onboard. The plane crashed about 12 NM (20 km) south of Madang, killing 28 occupants. 4 people onboard survived. The descent profile to Madang airport was steep because of the proximity of the Finisterre Ranges, a mountain range in north-eastern Papua New Guinea. The pilot-in-command was hand-flying the aircraft because the autopilot was unserviceable. He was maneuvering the aircraft visually to avoid cloud and thunderstorms. While descending, the overspeed warning horn sounded shortly followed by both propellers overspeeding simultaneously and exceeding their maximum rpm limit by about 60%. Seconds later, the crew declared an emergency and reported that both engines had stopped. The aircraft force-landed at 18 NM (33 km) south of Madang Airport, on sparsely timbered terrain on the north side of Buang River. The aircraft disintegrated due to impact with trees and ground and engulfed in flames. Rescuers rapidly localised the wreckage, which was on fire. The four survivors were the captain, the first officer, a flight attendant and a passenger.

Aircraft : CASA C-212-200
The Casa C-212-100 Aviocar operated by Nusantara Buana Airlines took off from Medan, Indonesia, for a passenger flight to Kutacane, Indonesia. 14 passengers and 4 crew were onboard. The plane crashed while on approach to Kutacane. All 18 occupants perished in that accident. The plane took off for a 35-minute flight. The plane went missing while on approach to its destination airport. Rescuers localized rapidly the wreckage on a steep slope of about 70 degrees at about 6000 feet of elevation (1800 meters), but as the plane crashed in a remote and difficult terrain, they were able to reach the wreckage only two days later. The plane was caught by the forest canopy as it crashed, leaving it lying on the jungle floor. The aircraft had sustained substantial damage to its front section as well as the aft section at the belly, further to the impact with the terrain at about 140 knots. However, there was no evidence of fire, and some parts of the aircraft (wings, centre section) are largely intact. Nevertheless, none of the aircraft doors had been opened, and all occupants were found dead in their seats. At the time of the accident, a cloud ceiling of 520 meters/1700 feet was reported.

Airline : Asiana Cargo
Aircraft : Boeing 747-48EF
The Boeing 747-400F operated by Asiana Cargo took off from Seoul, South Korea, for a cargo flight to Shanghai, China. Two crewmembers and 58 tons of cargo were onboard. The plane crashed into the ocean off Jeju Island, 67 minutes after it had taken off Seoul. The two people onboard were killed. The plane took off uneventfully, but then, while the B747 was enroute near Jeju Island (about 250 NM (463 km) South of Seoul), the crew reported the cargo in the hold had caught fire and they needed to divert to Jeju Airport, which was their only possible point of diversion. The crew's distress was clearly audible and increasing during the transmissions. The plane began to divert, but radar contact with the plane was lost while the plane was descending. Debris from the plane was found later in East China Sea, about 58 NM (107 km) west of the city of Jeju. The plane was completely destroyed. The cargo aircraft was carrying 58 tons of cargo including 0.4 tons of hazardous materials like Lithium batteries, paint, amino acid solution and synthetic resin. All the cargo onboard was compliant with the IATA (International Air Transport Association) regulation.

Airline : Omega Air Refueling
Aircraft : Boeing 707-321B
The Boeing 707-321B experimental aerial tanker plane, operated by Omega Aerial Refueling Services Inc, took off from Port Hueneme-Point Mugu NAS, CA, USA, for a test flight. The plane was expected to come back to its departure airport after the flight. 3 crewmembers were onboard. The four-engine plane crashed on takeoff, and has been destroyed. The three crewmembers sustained minor injuries. Omega Aerial Refueling Services Inc, was operating the airplane as a local public use flight. The flight crew indicated that they delayed their takeoff due to high wind conditions. Once the winds were within limits, they reported that startup was normal. Prior to take off, they decided to add 5 knots to their rotation speed, and power up slowly and as smoothly as possible. The captain was the flying pilot. The first officer called critical engine failure recognition speed (V1) reference speed at 141 knots, and rotation speed (Vr) at 150 knots. The captain rotated the airplane, and it then lifted off about 7000 feet (2130 meters) down the 11000-foot runway (3350 meters). About 20 feet (6 meters) above ground level, the number two engine (left inboard) throttle lever slammed back to the idle position. The airplane stopped climbing, and the captain adjusted the pitch slightly down to hold V2 speed. Witnesses observed the airplane lift off, and something shiny go up and over the left wing. They also observed a fireball on the left side of the airplane. The flight crew reported that the airplane began to drift to the left and descend. The captain lowered the pitch slightly and leveled the wings just prior to the airplane contacting the runway. He informed the other crew members that they were going to put it back down. He placed the throttle levers in the idle position, and activated the speed brakes. The airplane departed the left side of the runway surface; the crew reported a couple of impacts and then one final violent impact prior to the airplane coming to rest in a wetland marsh. The flight crew had to force the cockpit door open due to debris that had piled up against it. They noted significant damage from the forward galley aft, and a large fire in the vicinity of the left wing. They opened the left main cabin door, and deployed the slide. They exited into the mud, and made it to dry land and away from the burning wreckage. The airplane sustained substantial damage to the wings, fuselage, and empennage from impact forces and post crash fire. The main wreckage came to rest in a wetland marsh left of the runway overrun, and caught fire. Fire consumed the top of the cabin and the cockpit. The main wreckage consisted of the cockpit, cabin, right wing with the number three (right inboard) engine partially attached, empennage, and the inboard half of the left wing, which sustained thermal damage and was under water. Scattered debris aft of the main wreckage included the nose gear, remnants of the burned outboard left wing, right main landing gear truck, and number four (right outboard) engine.

Airline : Gulfstream Aerospace
Aircraft : Gulfstream G650
The Gulfstream G650 belonging to Gulfstream Aerospace (the Gulfstream manufacturer) took off from Roswell International Air Center Airport, New Mexico, United States of America, for a test flight. Four crewmembers (two pilots and two flight test engineers) were onboard. The plane crashed just after the lift-off, killing all the four occupants. The airplane was one of five G650’s engaged in flight testing as part of the model’s FAA and EASA certification program. The aircraft - operating as Gulfstream Test 31 - had spent the morning, approximately 2.5h, conducting takeoff-performance and brake testing. It was then just another takeoff: the plane was cleared for takeoff on runway 15. On its take-off roll the aircraft had just gotten airborne when the right wingtip struck the ground, causing the aircraft to lose altitude, collapsing the gear, skidding on the runway and catching fire. The plane skidded and came to a stop a few yards away from the control tower. The fire has completely destroyed the plane. Two Gulfstream pilots and two Gulfstream flight-test engineers died in the crash. The winds were under 10kts at the time of the accident.

Airline : Fugro Aviation Canada
Aircraft : CASA C-212-CC40
The CASA C-212-CC40 Aviocar 200 belonging to Fugro Aviation Canada took off from Saskatoon Airport, Canada, for a survey flight in the area of Saskatoon. Three people were onboard. During the flight, the plane developed engines problems and crashed while attempting to land. One person was killed, and two other injured. The twin-engine survey plane had been conducting geological survey work, usually done at an altitude of about 120 metres, near Saskatoon, when the pilots reported that one of their engines had failed. The pilots declared an emergency, and tried to return at Saskatoon Airport. They were on final approach to the airport, but it appeared they did not have enough power to reach the runway. The plane slammed into a sound proofing concrete barrier that separates a four-lane roadway from nearby homes. The plane came to abrupt stop, but no fire erupted. The plane was about 10 metres from a residential area and just across the street from a grocery store. One occupant (the technician) was killed and the pilot and co-pilot survived with light to moderate injuries. Nobody on ground was injured.

Airline : Trans Air Congo
Aircraft : Antonov AN-12BP
The Antonov AN-12 cargo, operated by Trans Air Congo, took off from Brazzaville, Congo, bound for Pointe Noire, Congo. Four crewmembers were onboard. The plane crashed while approaching Pointe Noire airport, killing all 4 people onboard, and 19 people on ground. The cargo plane had been largely empty, only carrying about 750 kg of beef. It slammed into a working-class residential area of Pointe-Noire port city as it was coming in to land, killing 23 people (including the 4 crewmembers and 19 fatalities on ground), and injuring 14 people on ground, and razing around a dozen houses and causing a fire that raged many hours. Images of the aircraft's last seconds show it diving steeply and rolling to the right, crashing inverted. The video appears to show exhaust trails coming only from the two port engines. It suggests the fatal roll was caused by the impossibility to counter with the rudder the loss of both engines on the same side. Pictures from the scene show the burnt-out remnants of the aircraft amid buildings in the suburbs of the city, with little remaining except the fin and tailplane.

Airline : Voronezh Aircraft Production Association
Aircraft : Antonov 148-100
The Antonov AN-148-100 was performing a training flight from Voronezh to Voronezh, Russia, with 6 crewmembers. The plane was en-route about 72 NM (134 km) South-South-West of Voronezh when the aircraft lost height and crashed in the village of Gorbuzovo Alekseevskogo. All 6 occupants perished in the crash. The cutting edge aircraft was about to be handed over to the Myanmar (Burma) Air Force. Two of the occupants were nationals of Myanmar and are believed to have been the pilots at the controls at the time of the crash. Just before the accident, the plane was en-route between the Voronezh and Belgorod regions. Eyewitnesses on the ground reported one of the wings and the tail separated from the aircraft and fall off. Officials confirmed finding debris of the aircraft about 1.5 NM (3 km) from the actual crash site, confirming the airplane began to break up in flight. Part of the wing and debris from inside the cabin has been located along the route from the first debris to the final crash site, none of that debris showing any evidence of fire. The plane has been destroyed by the impact on the ground and burst into flames.

Airline : Manx2
Aircraft : Swearingen 227
The Swearingen SA-227BC Metro III passenger plane, operated by Manx2, took off from Belfast, United Kingdom, for a passenger flight to Cork, Ireland. 10 passengers and two crewmembers were onboard. The plane crashed during landing at Cork. 6 occupants were killed, and 4 survived. On its first approach to Cork airport runway 17 the pilot aborted landing due to heavy fog. Then, he made a second attempt on Runway 35 in the opposite direction. Blinded by heavy fog he took the plane back into a holding pattern above the airport for 20 minutes. Then, the crew came back for a third approach, this time on runway 17. But the plane smashed nose first into the runway, overturned, careered at speed over the tarmac and ploughed through 70 ft of adjacent grass and mud. The right wing appears to have been crushed upwards in the impact, causing the engine on that side to burst into flames. With the plane imbalanced, its forward momentum forced it to flip over in a left-to-right motion, onto its roof. The front of the plane burst into flames but in less than two minutes two fire engines were at the side and had quickly extinguished the blaze. Both crewmembers and four passengers were killed. The four other passengers survived.

Aircraft : Tupolev TU-154B-2
The Tupolev Tu-154B-2, operated by Kogalymavia, was scheduled from Surgut, Russia, to Moscow, Russia. 116 passengers and 8 crewmembers were onboard. The plane has been destroyed by a fire while taxiing, just before takeoff. Three people died in this accident. The airplane had just been pushed back, the engines were being started when ground staff ordered an emergency shut down of the engines due to a fire that broke out in the rear of the aircraft between frames 62 and 65 (neither engines nor the APU were the source of the fire). The engines were shut down, the airplane de-powered and an evacuation was initiated. The fire reached rapidly the inside of the passenger cabin. Emergency services were able to extinguish the fire about 42 minutes after the fire broke out. A passenger said that the flames began to spread, especially after one of the passengers opened an emergency exit and air rushed in. The plane then quickly filled up with smoke that was black and acrid from burning plastic. 3 people died in this accident. 10 people were seriously injured, including six who were badly burned and four who suffered broken bones or other trauma. Most of the other injured passengers sought treatment for poisoning after inhaling toxic fumes.
